- We need to schedule you for an initial appointment. You can contact us by submitting the form on this website. If a phone call is easier, our office phone number is: (502) 581-1133. Our receptionist will set up your meeting with an attorney.
- If possible, please make a list of any doctors, clinics and hospitals you have visited over the past several months or since the date you became unable to work. It is always helpful to bring a list of all medications you take, or should be taking. Also, make a list of your past work history going back 15 years including the job title, company name, dates employed at said company and pay rate.
- All you have to do is show up. We will be ready to welcome you and learn about your specific situation.
How to Get to our office
Our office is located at 333 Guthrie Street, Ste 401, Louisville, KY 40202, which is just off of the east corner of 4th and Guthrie Streets. We are walking distance from 4th Street Live, the Seelbach Hotel and the Downtown YMCA. There are several bus lines in the area, along with on-street meter parking. We are in the Speed Building. Look for the maroon awning with “Speed A” on it. The elevator will bring you right up to our office on the 4th floor, Suite 401.
